IMPORTANT: Fill out the PDF form with your keyboard and your mouse before you print. You may scan it back to another PDF if you like, but we can only accept printed and signed HARD COPIES of the form with your failed device.Form will not print correctly from iPhone and iPad
If you download the form with an iPad or iPhone, please transfer the form to a MacBook, iMac, Windows PC/Laptop, or an Andriod device prior to printing the form, otherwise, the job number will not update.
Save your filled-out form in case it needs to be reprinted.
If you have no access to a printer, please email your completed form to This email address is being protected from spambots. You need JavaScript enabled to view it.. We can print it and have it ready for you when you drop off your failed device.
